Please inform us of your problem or condition so that we may plan your visit more efficiently. The more information we have, the better we will be able to prepare for your visit.
What should I bring for my first visit?
- Medicare Card
- Insurance Cards or Forms
- A list of all medications you take
- A list of any allergies to medication you may have
- Any materials you may have such as X-rays, CT, MRI, EMG's, blood work results, or other
- If you wear a brace, orthosis, or other supportive device, please bring it with you
- Date and history of your injury
What should I wear?
- It is preferable to wear loose and comfortable clothing
What should I not wear?
- Avoid wearing pants that are tight around the ankle
- Avoid wearing clothing that is bothersome to remove such as pantyhose or long underwear
- Avoid wearing cream on your feet for a couple of days before the examination
